In my experience, cheaper hotels charge per person and don't allow visitors at all; more expensive ones charge per room (though with a limit for visitors or extra guests).
IMO In reality this can only apply if you can't bypass the reception desk (e.g. a motel).
Additionally some states and countries require that all guests are registered and have shown some id, so if you wanna go up you've got to check in.
