The factory with the exploding fire this afternoon, also had a fire incident last year with a fatality. Appalling negligence by Worksafe.

Plenty. But unfortunately in this case it was already known that the owner was not operating a safe workplace. So once the fatality occurred worksafe would have investigated, given notices to fix the problems, and can order the place to shut down until issues fixed if the risks are high enough. Something has gone wrong with that process if the place caught fire again.
