...which would mean that you probably cannot actually tell how old someone is based on recognizing the good doctor.

... which also does make sense given the sacred ritual of the time warp.

Of course. I took my uncle to midnight showing, and the guy in front of him heard him disparaging the "hippies" around the theater. When Dr Frank-N-Furter came down the elevator, the guy stood up, and when Dr. F made his reveal, the guy spun around to face my uncle, and yanked his long coat open to reveal a full Dr F costume. It traumatized my uncle for life.
