I always figured this was for people with penises to attempt a urine trick shot into the second toilet while pooping.

This situation often means that the bathroom was remodeled, but there's not enough room to put in the wider ADA-compliant stall.
The ADA doesn't trigger on buildings until you do some sort of remodel. And moving or removing toilets is a much bigger job than some new flooring or whatever.
